Ages & Stages QuestionnairesÂ
Developmental screenings, known as the Ages and Stages Questionnaires (ASQ), are completed at every well-child visit from 2 months - 5 years of age.
These questionnaires are best completed on paper.

Please download, print, and complete the appropriate questionnaire, and bring it with you to your child’s visit.

If you forget to bring the completed ASQ, we are happy to provide one for you in the office.Â
