Ages & Stages Questionnaires

Developmental screenings, known as the Ages and Stages Questionnaires (ASQ), are completed at every well-child visit from 2 months - 5 years of age.

These questionnaires are best completed on paper.

Please download, print, and complete the appropriate questionnaire, and bring it with you to your child’s visit.

If you forget to bring the completed ASQ, we are happy to provide one for you in the office.

Know before you go: What to expect at each well-child visit

Congratulations on the birth of your new baby! For many parents, the first visit to the pediatrician is also their newborn's first trip away from home. Your child's first visit to our office is typically 1-3 days after they are discharged from the hospital.

What to bring:

Hospital discharge paperwork
Insurance cards (if you have them) and your ID

Screenings:

Review of hearing and heart screenings done in hospital

Development/Growth:

Weight, height, and head circumference measurements
Review of feeding and urine/stool output
Physical examination

Immunizations:

Hepatitis B vaccine if not already given in the hospital

Labs:

No routine labs, if baby is jaundiced they may need a bilirubin check

As you settle into life with baby, your pediatrician is keeping a close eye on their growth. We expect babies to back up or above their birth weight at the 2 week visit! You probably have many questions for your pediatrician, feel free to write them all down and we are happy to address each one!

What to bring:

Insurance cards (if you have them) and your ID

Development/Growth:

Weight, height, and head circumference measurements
Review of feeding and urine/stool output
Physical examination

Immunizations:

None, only Hepatitis B vaccine if not already given

Labs:

Newborn metabolic screen (heel prick done in-office)

Can you believe baby is already 1 month old? In addition to checking on your baby's developmental milestones, your pediatrician will also address what may feel like a million issues and questions!

What to bring:

Insurance cards (we expect all patients to have their insurance cards by the 1 month visit) and your ID

Screenings:

Parent will receive a mental health questionnaire

Development/Growth:

Weight, height, and head circumference measurements
Review of feeding and urine/stool output
Physical examination

Immunizations:

None

Labs:

No routine labs

It's time for your baby's 2 month visit! This is the first visit where baby will receive several immunizations. It may have you thinking, "Is it really safe to give my baby that many shots at once?" The answer is yes.

What to bring:

Insurance cards and your ID
A bottle of breastmilk/formula (if baby takes a bottle), as it's helpful to feed baby right after their shots!

Screenings:

Parent will receive a mental health questionnaire

Development/Growth:

Weight, height, and head circumference measurements
Review of feeding and urine/stool output
Ages and Stages (developmental milestone) evaluation
Physical examination

Immunizations:

Hepatitis B vaccine
Pentacel (combo of DTaP-Polio-HiB) vaccine
Pneumococcal vaccine
Rotavirus (oral vaccine)

Labs:

No routine labs

Your baby is getting bigger! They're also growing more interested in what you're doing. They may coo to get your attention. You may even hear a little chuckle if you try to make them laugh.

What to bring:

Insurance cards and your ID

Screenings:

Weight, height, and head circumference measurements
Review of feeding and urine/stool output
Ages and Stages (developmental milestones) evaluation

Immunizations:

Pentacel (combo DTaP-Polio-HiB) vaccine
Pneumococcal vaccine
Rotavirus (oral vaccine)

Labs:

No routine labs

Happy half-birthday to your beautiful baby! By 6 months, most babies have doubled their birth weights.

What to bring:

Insurance cards and your ID

Screenings:

Option during flu season: Flu Vaccine (note: infants receiving their flu vaccine for the first time will need 2 doses separated by 1 month)
